1. Acceptance of Terms
The following “Terms of Service” govern your use of the Thrive Heroes Foundation website (the “Site”). This is a binding agreement between you and Thrive Heroes Foundation (“THF,” “we,” “us,” or “our”) and incorporates our Privacy Policy.
Thrive Heroes Foundation is a nonprofit corporation registered in the Commonwealth of Virginia and recognized by the Internal Revenue Service as a tax-exempt charitable organization under Section 501(c)(3) of the Internal Revenue Code, EIN 41-2572498.
By accessing, using, applying to receive support, donating, or participating in any Thrive Heroes Foundation program or service (the “Services”), you agree to be bound by these Terms.
We reserve the right to modify or update these Terms at any time. Continued use of the Site or Services constitutes acceptance of updated Terms.
As used in these Terms, references to our “Affiliates” include our board of directors, donors, volunteers, partner organizations, contractors, vendors, and all parties engaged in supporting our programs.

2. Services Provided
Thrive Heroes Foundation is a charitable nonprofit organization providing access, resources, assistance programs, and support services to qualifying military personnel, veterans, first responders, and select community groups based on available donations and funding capacity.
Thrive Heroes Foundation is not a healthcare provider and does not deliver chiropractic care, medical services, diagnosis, or treatment. All healthcare services are provided solely by independently licensed healthcare professionals operating under Virginia law.
a. No Guarantee of Support
Services and assistance are dependent upon donations and available funding. Applying does not guarantee approval for support. Selection may be based on need, eligibility, available resources, and funding availability at the time of application.
b. Temporary Interruption
Access to our website, application forms, or communications may be temporarily interrupted. Services are provided “AS IS.”
c. Right to Modify Services
We may modify or discontinue Services, programs, eligibility rules, availability, or offerings at any time without notice.
d. No Contingency or Entitlement
Applying or communicating with us does not create any entitlement or expectation of future funding, support, or medical services.
3. Donations and Payments
Donations made to Thrive Heroes Foundation are voluntary, irrevocable charitable contributions and are non-refundable except as required by law.
Recurring donations may be terminated by written notice but funds already processed are generally non-refundable.
Charitable contributions are tax-deductible to the extent permitted by law. No goods or services were provided in exchange for a contribution unless expressly stated in writing.
4. Conduct
Users agree not to post or transmit any unlawful, defamatory, harassing, discriminatory, threatening, fraudulent, or harmful material on our Site, social media, or communication channels.
We may remove any content at our discretion.
5. Thrive Heroes Foundation Intellectual Property
All content on the Site—including text, graphics, logos, images, educational materials, documents, trademarks, and original content—is the property of Thrive Heroes Foundation or affiliated creators and may not be copied or reproduced without permission.
You may download information for personal use only.
6. User Submitted Content
Any content submitted by a user—such as testimonials, inquiries, or materials voluntarily provided—may be used by Thrive Heroes Foundation for educational, informational, or promotional purposes, unless you specifically request confidentiality in writing.
7. Data and Privacy
All personal information collected is governed by our Privacy Policy. We are committed to maintaining high standards of data protection and confidentiality. However, no online platform is 100% secure, and data transmission is at your own risk.
Thrive Heroes Foundation may receive or review health-related information for the limited purpose of determining eligibility and coordinating sponsorship. Thrive Heroes Foundation is not a HIPAA covered entity, but treats such information confidentially and in accordance with applicable federal and Virginia law.
8. Eligibility
Eligibility for programs or services is determined solely by Thrive Heroes Foundation. Submitting information does not guarantee approval, medical services, or financial assistance.
No applicant acquires any legal right, property interest, or entitlement to financial assistance, treatment, or program participation from Thrive Heroes Foundation. All eligibility decisions are final and not subject to appeal.
9. Disclaimer
ALL SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE.” THRIVE HEROES’ FOUNDATION MAKES NO GUARANTEE THAT YOU WILL RECEIVE SUPPORT, MEDICAL SERVICES, OR FUNDING.
THF does not provide medical advice. Any health-related services are performed by licensed third-party providers and are subject solely to their policies and legal requirements.
All clinical decisions, diagnoses, treatment recommendations, and medical outcomes are made solely by independently licensed healthcare providers and not by Thrive Heroes Foundation.
10. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, THRIVE HEROES’ FOUNDATION SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF USE OF OUR SITE OR SERVICES.
Your sole remedy for dissatisfaction with the Site or Services is to stop using them.
11. Indemnification
You agree to indemnify and hold harmless Thrive Heroes Foundation and its Affiliates from any claims, demands, damages, costs, or liabilities arising from your use of Services, your breach of these Terms, or reliance on information provided by Thrive Heroes Foundation or its partners.
12. Termination
We may suspend or terminate access to the Site or Services at our discretion, including circumstances involving misuse, fraud, inappropriate conduct, or violation of these Terms.
13. Miscellaneous
a. Privacy
Personally identifiable information is handled pursuant to our Privacy Policy.
b. Governing Law
These Terms are governed by the laws of the Commonwealth of Virginia.
c. Healthcare Licensing Notice
Any services provided through Thrive Integrative Health or other participating clinics are rendered by independently licensed healthcare professionals operating under the laws and licensing requirements of the Commonwealth of Virginia.
d. Severability
If any provision is determined unenforceable, the remaining provisions remain in effect.
e. Entire Agreement
These Terms constitute the entire agreement relating to your use of our Site and Services.

14: REQUEST SUPPORT DISCLOSURE
Thrive Heroes Foundation helps reduce barriers to care by providing financial sponsorship and access support. Any services are delivered by independent healthcare providers such as Thrive Integrative Health in Springfield, Virginia.
This form is not for emergencies. If you are experiencing a medical emergency, call 911.
Approval depends on funding and capacity. Applicants may be placed on a waitlist. Selection is based on need, service history, clinical appropriateness, financial circumstances, and program capacity. Decisions are final and non-appealable.
Submitting a request does not guarantee any form of support or medical services.
15: MEDICAL SERVICE DISCLAIMER
Thrive Heroes Foundation does not provide medical treatment and does not practice chiropractic, medicine, or any licensed healthcare profession.
Treatment decisions are made exclusively by licensed professionals and are not controlled by THF.
Nothing in this policy substitutes professional medical advice. If you have medical questions, consult a licensed provider. In emergencies, call 911.
16: HIPAA AND PRIVACY NOTICE
Thrive Heroes Foundation is not a covered entity under HIPAA. However, information you provide may include personal or health data.
By submitting a request, you authorize THF to share the information you provide with participating providers for eligibility review and coordination of sponsorship.
Participating providers are responsible for HIPAA compliance and will provide their own Notice of Privacy Practices.
17: DONOR LEGAL STATEMENTS
Thrive Heroes Foundation is a 501(c)(3) tax-exempt organization, EIN 41-2572498. Contributions are tax-deductible to the fullest extent permitted by law.
Unless otherwise stated, no goods or services were provided in exchange for donations.
Restricted donations are accepted as preferences but are used at the discretion of THF.
Recurring donors authorize charges until cancelled.
18: SPONSORSHIPS
Sponsorships may include recognition but do not imply endorsement, ownership, or control. Sponsorship funds are donations and used at THF’s discretion.
19: ELIGIBILITY OVERVIEW
Support generally focuses on active-duty military, veterans, first responders and qualifying groups.
Clinical decisions, medical judgment, and treatment plans are the responsibility of licensed providers.
